1. Tape Submission Form - fill this out for each order and either send with tapes or fax it to us.

These forms are in the PDF() format and require Adobe Acrobat Reader. It can be downloaded for free here if your machine does not already have it installed. Most new Windows and Macintosh systems come with Acrobat pre-installed.

Before sending us materials to transcribe you must sign up for an account. No work will be performed without an account.

Physical Tapes, VHS, DVD, Mini Discs, CDs, etc should be sent along with a completed submission form to:
